Sports Injury Fast Track Program

If you have a referral from your primary care physician, please book your AESM appointment by clicking the link below, or call us at 905-707-6880.

Patients must be referred by a physician to receive an appointment via the fast track program. If your physician / family doctor has not yet sent this referral, simply have him or her download this AESM Sports Injury Referral Form https://www.aesmphysiotherapytoronto.ca/wp-content/uploads/2021/04/Referral-Pad-RH-6-x-1-Apr9-2021.pdf, with the Fast Track box checked off, and fax it to  905-707-6880.
