Fast Track ProgramIf you have a referral from your primary care physician, please book your AESM appointment by clicking the link below, or call us at 416-800-0800.

Patients must be referred by a physician to receive an appointment via the fast track program. If your physician / family doctor has not yet sent this referral, simply have him or her download this AESM Sports Injury Referral Form, with the Fast Track box checked off, and fax it to 416-800-0802.
